I have no official word to back this up, but he's been sacked 4 times and fumbled twice, so I wouldn't be surprised if he got a concussion or maybe hurt his hand to the point where he couldn't grip the ball.
You don't pull David Carr unless he's hurt.

David Carr was replaced by Sage Rosenfels midway through the third quarter of Sunday's game at Tennessee. Before being pulled, Carr completed 15-of-21 passes for 113 yards, no TDs, and an INT. Carr killed Houston's promising first drive by throwing a pick into quadruple coverage. He had a ball batted before throwing another in the dirt deep in Titans territory during the second quarter. To close the half, Carr was sacked, fumbled, and the ball was returned for a TD. Carr lost another fumble to kill yet another promising drive early in the second half. We doubt a QB controversy in on the horizon for Houston.

meanwhile, Rosenfels is 9-for-15 with 118 yards, 1 int, and TWO TDs, so apparently, it was a decent call...
